PVAO Pension Loan Requirements

AFP PVAO - Must be an existing client of Silver Finance
PVAO Form (any 1 of the 3)
White Form (for Old Age Pensioner - Veteran)
Pink Form (for Surviving Spouse)
Blue Form (for Death Pension)
PVAO ID Card
ATM Card (to be turned over for collection purposes)
Birth Certificate - PSA Certified true copy
Two (2) pieces 2x2 Picture
Two (2) Other Valid Government issued ID's (SSS, Driver's License, NBI Clearance, Passport)
PSA issued Marriage Certificate (for widows) - PSA Certified true copy
Form#5 Advisory of Marriages (for surviving spouses) - PSA form
Death Certificate
Bank Statement
ATM/POS Transaction Receipt as verified by Credit Through Collection Department (to confirm the card is activated)
